The result depends almost entirely on how the PDF was created. A PDF exported from Word, Google Docs or a similar program contains a real text layer, and converting it back to DOCX usually gives you clean, fully editable text. A PDF produced by scanning a paper document contains only an image of the text, so unless optical character recognition is applied you will get a Word file containing a picture rather than words you can edit. Complex multi-column layouts, heavy tables and text boxes are the most likely things to come across misaligned.
A few habits noticeably improve the output. If you only need part of a long document, extract those pages first, because conversion accuracy tends to drop over very long files. Expect to rebuild tables by hand, since column boundaries are inferred rather than stored. After converting, turn on formatting marks in Word to find the stray section breaks and text boxes that conversion tools tend to leave behind, and clear them before you start editing properly. Fixing the structure first is much faster than fighting it while you write.
If your only goal is to pull a few paragraphs out, opening the PDF and copying the text directly is faster and cleaner than a full conversion. If you need to sign, annotate or comment, most PDF readers do that natively and converting will only cost you the layout. And if the document is a legal or official record, converting it to Word breaks its fixed formatting and may make it unacceptable to the body that issued it. Convert when you genuinely need to rewrite the content, not when you simply need to read or mark it.
